Medical Forms

Should your child require a prescribed medicine during school hours, please complete the Prescribed Medicine form.  

 

If a Non prescribed medicine is required, please compete the

Short Term Non Prescribed Medicine form.  Please be aware that there are some non prescribed medicines that we are unable to administer, for clarification please contact the school. 

 

We require forms to be completed and handed into the school office, If you are unable to print these forms, we have hard copies available.
