Should your child require prescribed medication during school hours, please complete the Prescribed Medicine form.  All prescribed medicines must be in their original packaging, which must include the pharmacist's prescription label.

If a non-prescribed medicine is required, please complete the Short Term Non-Prescribed Medicine form.  Please note that there are some non-prescribed medicines that we are unable to administer.  Please contact the school office for clarification and advice.

All forms must be completed and handed into the school office.

If you are unable to print and complete the attached forms, we have hard copies available for completion at the school office.
